Insurance Information

Our office will provide you with the finest care and the most convenient financial options. To accomplish this, we work with you to maximize your insurance reimbursement for covered procedures. Our office is in network for most PPO insurances and Medicaid/Hoosier Healthwise. 

We will collect your insurance information on the day of the appointment and, as a service to our parents, we will be happy to file your claim for you. We file all insurance electronically so your insurance company. We ask that you are familiar with your insurance benefits, as we will collect from you the estimated amount insurance is not expected to pay. By law your insurance company is required to pay each claim within 30 days of receipt. You are responsible for any balance on your account after 30 days, whether insurance has paid or not. We will send a refund to you if your insurance pays us.

We do not have a contract with your insurance company, only you do. We will assist you in estimating your portion of  the cost of treatment, and for detailed information we will provide you with the appropriate codes relating to your child’s care. You can then contact your insurance company directly or we can submit a pretreatment estimate on your behalf. We are unable to guarantee what your insurance company will or will not do with each claim.

It is important that you keep us informed of any insurance changes such as policy name, insurance company address, or a change of employment.
